ABSTRACT

Objective:To analyze the clinical features of patients with severe dengue (SD) in Guangdong Province, and to improve the understanding of the diagnosis and treatment of SD in China.Methods:The clinical data, laboratory examination and etiological test results of 257 SD cases from 29 dengue fever designated hospitals in Guangdong Province from January 1, 2013 to December 31, 2019 were respectively collected. The relevant indicators of the criteria for severe organ involvement were quantified. Logistic regression analysis was performed to analyze the risk factors for the development of multiple organ failure in SD patients.Results:Among the 257 SD patients, age was (64.1±20.1) years old, with 65.4%(168/257) of them ≥60 years old, 142 were male and 115 were female. One hundred and fifty-two (59.1%) patients had underlying conditions, including 115(44.7%) patients with hypertension. The clinical manifestations were mainly fever (98.4%(253/257)), fatigue (70.0%(180/257)), cough or expectoration (44.4%(114/257)), lethargy or irritability (39.3%(101/257)), vomiting (30.4%(78/257)), abdominal pain or tenderness (20.6%(53/257)), hepatomegaly (2.3%(6/257)), bleeding tendency (59.5%(153/257)), and pleural effusion or ascites (43.6%(112/257)). Platelet count levels were decreased in 90.9%(231/254) of the cases, and 97.1%(234/241) of patients had normal or decreased hematocrit. The most common of severe manifestations were severe organ involvement (61.1%(157/257)), followed by severe bleeding (37.0%(95/257)) and severe plasma leakage (30.0%(77/257)). Severe organ involvements were more common in the kidney (27.6%(71/257)) and heart (26.8%(69/257)). Multivariate logistic regression analysis showed that age (odds ratio ( OR)=1.051, 95% confidence interval ( CI) 1.004 to 1.100, P=0.035), hypertension ( OR=5.224, 95% CI 1.272 to 21.462, P=0.022), elevated aspartate aminotransferase (AST) level ( OR=1.002, 95% CI 1.001 to 1.003, P=0.001), blood urea nitrogen (BUN) ( OR=1.050, 95% CI 1.005 to 1.098, P=0.030), and international normalized ratio (INR) ( OR=4.604, 95% CI 1.601 to 13.238, P=0.005) were risk factors for the development of multiple organ failure in SD patients. The detection results of serum samples form 113 SD patients in acute phase showed that dengue virus (DENV)-1 accounted for 89.4%(101/113), DENV-2 accounted for 9.7%(11/113), and DENV-3 accounted for 0.9% (1/113). Conclusions:Elderly and those with co-existing conditions such as hypertension in SD patients in Guangdong Province are more common. Severe organ involvement such as kidney and heart is the main cause of SD. DENV-1 infection is predominant. Significant elevated levels of AST, BUN and INR may be related to a poor prognosis.

ABSTRACT

OBJECT IVE To evaluate the application effect of the whole cour se medication management mode led by pharmacists in rheumatic immune diseases. METHODS A total of 122 patients treated with tacrolimus or cyclosporine in the department of rheumatology and immunology of Wuhan No. 1 Hospital from 2018 to 2020 were selected as the study subjects. Among them ,44 cases in the control group were under the traditional supervision mode ;78 patients in the observation group adopted the whole course medication management mode led by pharmacists ,that was ,individual pharmacists and specialist clinical pharmacists cooperated and led ,and not only participated in the whole process of drug treatment but also involved in the whole process of therapeutic drug monitoring (TDM). On the basis of the control group ,the division of labor and cooperation among medical,pharmaceutical and nursing parties were strengthened ,and the homogeneous supervision was carried out for the outpatients and inpatients from admission to discharge . The daily dose of medication ,the rate of reaching the standard of blood drug concentration ,the incidence of problematic samples (the sample was calculated by the number of times ),the average hospitalization days ,the re-admission rate within 6 months after discharge ,the medication compliance score and the patient ’s satisfaction rate were compared between the two groups. RESULTS In the control group ,53 times of TDM were performed , including 18 times of tacrolimus monitoring and 35 times of cyclosporine monitoring ;in the observation group ,123 timesof TDM were performed ,including 55 times of tacrolimus monitoring and 68 times of cyclosporine monitoring. The daily dose of tacrolimus ,the daily dose of cyclosporine ,the rate of reaching the standard of cyclosporine blood drugconcentration,the inc idence of problematic samples ,the rate of re-admission within 6 months after discharge , the medication compliance score and the patient ’s satisfaction rate in the observation group were significantly better than those in the control group (P<0.05). CONCLUSIONS It can effectively improve the effect of the quality of pharmaceutical care to implement whole course and homogeneous medication management led by pharmacists and provide precise drug guidance for patients with rheumatic and immune diseases.

ABSTRACT

ObjectiveTo observe the clinical effect of Xijiao Dihuangtang combined with acupuncture on the treatment of systemic lupus erythematosus (SLE) with syndrome of intense heat toxin. MethodA total of 102 cases of SLE with syndrome of intense heat toxin (admitted from January 2020 to December 2021) were divided into a control group and an observation group by random number table method, with 51 cases in each group. Patients in the control group received conventional western medicine treatment (hydroxychloroquine sulfate + poonisone acetate tablets). Patients in the observation group were given Xijiao Dihuangtang combined with acupuncture based on the treatment of the control group. Clinical efficacy, onset time, symptom improvement time, skin lesion regression time, and hospital stays were recorded. Blood routine index change, 24 h urine protein levels, double-stranded DNA (ds-DNA) antibodies resistance index changes, traditional Chinese medicine (TCM) syndrome integral changes, and changes in serum cytokine indexes, blood rheology indexes, and T lymphocyte subsets indexes were observed. SLE changes in disease activity index (SLEDAI-2000), quality of life (SF-36) scores, and adverse reactions were explored. ResultThe total effective rate was 96.08% (49/51) in the observation group, higher than 84.31% (43/51) in the control group (χ2=3.991, P<0.05). The onset time, symptom improvement time, skin lesion regression time, and hospital stays in the observation group were shorter than those in the control group (P<0.05,P<0.01). Before treatment, erythrocyte sedimentation rate (ESR), platelet count (PLT), hemoglobin (HGB), white blood cell count (WBC), and other blood routine indexes, 24h urinary protein levels, anti-ds-DNA, TCM syndrome integral core, serum cytokine indexes including interleukin-4 (IL-4), IL-6, soluble interleukin-2 receptor (sIL-2R), blood rheology indexes (fibrinogen, plasma viscosity, and whole blood viscosity),T lymphocyte subsets indexes (CD4+, CD8+, and CD4+/CD8+), SLEDAI-2000, and SF-36 score comparison in both groups had no statistical significance. After treatment, the above indicators were improved in each group (P<0.05), and the indicators in the observation group was better than that in the control group (P<0.05). In terms of the adverse reactions, there was 1 case of digestive tract reaction, 1 case of elevated aminotransferase, and 1 case of insomnia in the observation group. There were 2 cases of digestive tract reaction,1 case of infection, and 1 case of elevated transaminase in the control group. The difference of the adverse reactions between the two groups had no statistical significance. ConclusionXijiao Dihuangtang combined with acupuncture has good clinical effect on the treatment of SLE with syndrome of intense heat toxin, and the method is safe, reliable, and worthy of application.

ABSTRACT

Anthocyanins are widely distributed water-soluble pigments that not only give the fruit colorful appearances, but also are important sources of natural edible pigments. In recent years, the interest on anthocyanins of solanaceous vegetables is increasing. This paper summarized the structure of anthocyanins and its biosynthetic pathway, the structural genes and regulatory genes involved in the biosynthesis of anthocyanins in solanaceous vegetables, as well as the environmental factors affecting the biosynthesis. This review may help clarify the synthesis and regulation mechanism of anthocyanins in solanaceous vegetables and make better use of anthocyanins for quality breeding of fruit colors.

Objective@#To analyze the clinical features of death cases of dengue fever and the causes of their deaths.@*Methods@#The clinical data and death reports of nine death cases of dengue fever in Guangdong Province from June 23, 2014 to September 10, 2019 were retrospectively analyzed. All of nine cases were positive for serum dengue virus RNA as confirmed by reverse transcription polymerase chain reaction, and some of the virus strains were serotyped.@*Results@#The median age of the nine patients was 57.5 (range: 18-80) years. Among them, six patients were females; eight patients were local cases and one was imported case; all of nine cases occurred in September and October. The median time from onset to visit hospital and diagnosis was three and four days, respectively. Three of the nine patients had underlying diseases. All of nine cases had fever, including three with double-peak fever. Eight of the cases had three or more severe dengue fever warning indexes before admission. Three cases had severe bleeding upon admission, one case had shock, and six cases had organ failure. Three cases underwent invasive examination, including arterial puncture catheterization, endoscopic titanium clip hemostasis, and percutaneous transluminal coronary angiography. Two cases developed malignant arrhythmia and one had massive hemorrhage of subcutaneous soft tissue after operation. The death cases included four cases of cardiogenic shock and fatal arrhythmia, three cases of encephalitis and encephalopathy, one case of refractory shock, and one case of acute liver failure. Among the six cases that underwent serological typing, four were identified as dengue virus type 1 (DEN-1) and two were DEN-2.@*Conclusions@#Severe organ failure is the major cause of dengue fever-related death, especially fulminant myocarditis, and DEN-1 is most common. Early diagnosis and treatment, and avoidance of invasive procedures can effectively reduce the mortality rate of the severe dengue fever patients.

ABSTRACT

Objective To investigate the value of F-actin autoantibodies in the serum of patients with systemic lupus erythematosus (SLE),and to explore the relationships between F-actin autoantibodies and other clinical indicators.Methods ELISA was established to detect serum levels of F-actin autoantibodies in 93 inpatients with SLE from March 2017 to January 2018 (case group,n=93),72 patients with rheumatoid arthritis (RA) (disease control group) and 83 healthy subjects (healthy control group) were included during the same period.The positive rates of F-actin autoantibodies between the case group and the two control group were compared.Clinical data including SLE disease activity index (SLEDAI),immuno-globulin (lg)G,erythrocyte sedimentation rate (ESR),anti-dsDNA,and antinuclear antibody (ANA) of 93 patients with SLE were collected and the correlation analysis between F-actin autoantibodies units was applied respectively.The diagnostic performance of F-actin autoantibodies in SLE was analyzed by using the receiver operating characteristic curve (ROC).T test,Chi-square test and Spearman/Pearson correlation analysis were applied for statistical analysis.Results The serum levels of F-actin autoantibodies in the SLE case group,disease control group,and healthy control group were (18±13),(12±6),and (11±5) U,respectively,the differences between SLE case group and disease control group,and healthy control group were significant (t=3.163,P=0.001 9;t=4.436,P<0.01).The positive rates of F-actin autoantibodies were 33%(31/93) in patients with SLE,10%(7/72) in disease control group,and 4%(3/83) in healthy control group.The F-actin autoanti-bodies units in SLE were correlated with SLEDAI,IgG,ESR,anti-dsDNA,and ANA (r=0.273 7,P=0.008 3;r=0.558 7,P<0.01;r=0.419 9,P=0.000 1,r=0.351 4,P=0.001 1,r=0.460 9,P<0.01),in which F-actin autoantibodies units showed significant correlation with IgG and ANA.In the ROC curve,the area under the curve(AUC) was 0.62 [95%CI(0.54,0.70)],P=0.001 3.which was statistically significant.When the cut-off value of the F-actin autoantibodies was 14.04 U,the Youden's index (YI) was the largest (YI=0.30),and the sen-sitivity for the diagnosis of SLE was 0.77,the specificity was 0.53.Conclusion The positive rate of F-actin autoantibodies in the serum of patients with SLE is higher than that of RA and healthy controls,so it has certain diagnostic value for SLE.The F-actin autoantibodies units is correlated with both SLEDAI,ESR,and anti-dsDNA,suggesting that F-actin autoantibodies units may be a new biomarker for disease activity assessment of SLE patients.

ABSTRACT

To explore the treatment method and preventive measures on epidemic keratoconjunctivitis. 108 patients with epidemic keratoconjunctivitis who received treatment in our hospital from January, 2015 to September, 2015.were selected. These patients were treated with interferon eye drops, Ganciclovir ophthalmic gel, and alternating eye treatment of tobramycin-dexamethasone eye drops and diclofenac sodium eye drops. Meanwhile, health education was also performed among patients, so as to promote the recovery of the disease as soon as possible and to prevent the spread of the disease Among the 108 patients, there were 101 patients recovered. 7 patients had cornea remained sub epithelial round hoary haze, including 2 patients with evident cornea remained sub epithelial round hoary haze due to the occurrence of glucocorticoid-induced intraocular pressure and the tobramycin and dexamethasone eye drops were suspend. The clinical cure rate was 91.79%. There was no pathophoresis to health patients among the 108 patients. Active treatment of epidemic keratoconjunctivitis, combined with health education and publicity could increase the clinical cure rate and control the transmit of the disease spread

ABSTRACT

Objective:To revise the Chinese version of Devereux Early Childhood Assessment for Preschool-ers,Second Edition (DECA-P2 ),and assess its validity and reliability.Methods:Totally 608 children aged 3 -5 years were rated by their teachers with the Chinese version of DECA-P2 and two criterion scales,which included the Chinese version of Preschool Behavioral and Emotional Rating Scale,and Social Competence and Behavior Evalua-tion Scale-30.Data of 570 were valid.After identifying valid items using item analysis,exploratory factor analysis was performed for half of all samples.Confirmatory factor analysis was also conducted with another half samples.Moreover,the present study tested criterion-related validity,internal consistency reliability and split-half re-liability of the Chinese version of DECA-P2.Results:Through the exploratory factor analysis,the obtained structure of protective factors was the same as the original scale,accounting for 59.82%of the variance.The results of con-firmatory factor analysis achieved the standard of psychometrics (χ2/df=2.50,RMESA=0.07,CFI=0.91,IFI=0.91).Generally,each subscale had significant correlation with the two criterion scales (r=-0.21 ~0.80). Be-sides,the Cronbach αcoefficients of all subscales were between 0.80 and 0.93,and the split-half reliabilities of them were between 0.79 and 0.89.Conclusion:The Chinese version of DECA-P2 has acceptable psychometric quality and can be applied to evaluate resilience for children aged from 3 to 5 years in China.
